FAQs

Neuropsychological Evaluation Process

On average, each evaluation takes anywhere from 8 to 15 hours. There is a 60-90 minute intake session to review the in-depth intake packet (completed and submitted before the first appointment) and discuss referral concerns and relevant background information.

Intake and feedback sessions can be completed via telehealth based on patient preference; however, testing is completed in person and designed specifically for you.

The testing length is determined by the number and type of evaluation measures required to address the referral concern. The goal is to complete the testing process in one day; however, occasionally, testing will need to be separated into more than one session. These details will vary from person to person.

Questionnaires and surveys help us to understand the patient outside of the clinical setting. These are usually completed by the patient, family, and/ or other collateral sources (such as teachers or school personnel). There is a 60-minute feedback session to review the results and recommendations.

A full report with strengths, vulnerabilities, and recommendations is provided, typically one to two weeks after the feedback session. This report can also be sent to any additional team members as long as a release of information is signed.

Psychiatric Medication Management Process

There is a 60-minute intake assessment to review the intake packet (completed and submitted before the first appointment) and discuss referral concerns and relevant background information. Additional 30-minute follow-up appointments are scheduled based on the unique needs of each patient.

Individual Therapy Process

An initial diagnostic assessment to determine individual needs for therapy and overall fit. Additional therapy appointments will be scheduled based on the unique needs of each patient. The duration of therapy can vary based on the referral concern.

What age range do you serve? 

Dr. Lee provides comprehensive neuropsychological services for patients across the lifespan (children ages six and up, adolescents, adults, and older adults).

Emily Luitjens provides psychiatric medication management services for patients ages 14 and up.  She specializes in working with older adults and individuals diagnosed with dementia.

Carrie Gapinski is licensed to see individuals across the lifespan but specializes in children and adolescents.

How do I schedule an appointment? 

You can contact the clinic via the contact link on the website, or you can call the clinic directly (320-247-4068).  A referral link on the website is also available.
 
Outside clinic referrals can be sent via fax (218-656-7265).
 
If agencies (county, school districts, clinics, etc) are interested in discussing contracting with Clary Clinic for services, contact Dr. Lee.

What is the current wait time for appointment availability?

Neuropsychological evaluations are currently scheduled for initial appointments within one to two months.  
 
Psychiatric medication management and individual therapy services are currently scheduled for initial appointments within one month.

Do you accept insurance?

Emily Luitjens and Jessica Lee are Clary Clinic providers that accept Medicare Part B for neuropsychological evaluation and psychiatric medication management coverage. They do not currently accept other insurance reimbursement.

Other Clary Clinic providers do not file for insurance reimbursement. You pay the clinic directly for services. Most of our services are considered “out of network” due to inconsistent and low insurance reimbursement. Your insurance provider may still cover out-of-network services. We will happily provide you with a superbill after the evaluation, which can be provided to your insurance provider for potential reimbursement. Most patients receive 50-80% insurance reimbursement for services.
 
Why out of network?
Our goal is to provide you with mental health services that are comprehensive, individualized, and timely. We want decisions about your care to be made by you and your provider, not insurance employees. We want to complete the assessments that we feel are medically best, not just ones more likely to be paid by an insurance company. Insurance companies require a diagnosis and make it a permanent part of the patient’s medical record. Our patients should be able to choose if they want to do that.
 
Insurance companies do not cover testing for learning and attentional concerns, as well as many other areas that are often necessary to include in comprehensive neuropsychological evaluations. Although the insurance company will indicate the evaluation or service is covered, it has generally left the clinic/ provider without compensation.  Out-of-network services are a way for us to maintain a high standard of care in a financially sustainable way.

What is a superbill?

A superbill is a record of all the procedures (CPT codes) that were used as part of the evaluation along with any diagnoses (ICD-10 codes) made based on the results. Superbills are different from reciepts or invoices because they provide additional information regarding the visit (diagnoses and procedure codes) required to have a claim approved. If the patient is able to be reimbursed for a portion or all of the evaluation costs, we will assist you in understanding the superbill.

How does payment for services work?

Cash and debit/credit cards are accepted forms of payment. Some HSA/ FSA accounts also allow for payments out of those accounts, which is also an accepted form of payment. If you have a pre-tax account set aside for healthcare expenses, you can use the debit card associated with that account. You will be asked to provide a credit or debit card on file in our clinic during your scheduling.
 
Fees for psychiatric medication management and individual therapy will be due at the time of or before the scheduled appointment. 
 
Due to the high demand for neuropsychological evaluation appointments and the extensive nature of the appointment, evaluation appointments, half of the full fee ($1,450) is required at the time of scheduling to reserve your appointment. The remaining balance of $1,450 is due on the testing appointment date. If you cannot make it to your evaluation appointment, please contact our office to cancel or reschedule at least 48 hours in advance. Please keep in mind that appointments canceled 48 hours or more ahead of the scheduled appointment will forfeit $100 of the initial fee.  If you cancel less than 48 hours before your appointment or do not show up, $300 of the initial fee will be forfeited.

Rates & Insurance

Rates

Neuropsychological evaluations are a one time investment of $2,900, which includes a clinical interview, test administration and scoring, test evaluation services (interpretation/ integration of test results and clinical data, clinical decision-making, and report writing), and feedback appointment. 

If out of network, psychiatric medication management services are an investment of $500 for the initial intake assessment, $250 for 30-minute follow-up appointments, and $125 for 15-minute follow-up appointments.

Individual therapy services are an investment of $150 per 50-minute session. 

Additional services, such as completion of forms or preparation of letters on behalf of the patient, consultative services, telephone calls with the patient or other treating providers regarding the patient and their evaluation is complete lasting longer than 10 minutes, and time spent performing other miscellaneous services requested by the patient will be billed at $250 per hour prorated.

Insurance

Clary Clinic accepts Medicare Part B for insurance reimbursement. The clinic is not currently accepting other insurance reimbursement.

Most services at Clary Clinic are considered out-of-network. Depending on your current health insurance provider or employee benefit plan, services may be covered in full or part. Please contact your provider to verify how your plan compensates you for neuropsychological and psychiatric services.

  • What are my out-of-network benefits?
  • How much is my deductible?
  • After I meet my deductible, what percentage of the visit cost will I be reimbursed for?
  • For neuropsychological evaluation, ask up to how much can I be reimbursed for:
    • 60-minute intake appointment (CPT Code 90791)
    • Neuropsychological Testing (CPT Code 96136 for the first 30 minutes, CPT Code 96137 for each additional 30 min)
    • Neuropsychological Services (CPT Code 96132 for the first 60 minutes, CPT Code 96133 for each additional hour)
    • 60 minute feedback session (CPT Code 96133)
  • Do I have a limit on how many units of 96137 or 96133 I can get reimbursed for?
  • How do I submit a claim with the receipt my doctor gave me?

Payment

Clary Clinic accepts cash, checks, and all major credit cards as forms of payment. HSA/ FSA payments are also accepted.  Clary Clinic is also a CareCredit provider.  You can find additional information about CareCredit here.
