Maybe you’ve had evaluations before that left you with more questions than answers. Maybe you just need someone to take the time to really understand what’s going on. What to Expect:
The Evaluation Process
Step 1 — Intake Session (60–90 minutes)
Before your evaluation, you'll complete an in-depth intake packet. At the intake session — available via telehealth or in person — Dr. Lee reviews your background, listens to your concerns, and makes sure the evaluation is tailored specifically to you.
Step 2 — Testing
Testing is conducted in person and tailored to your individual needs. We schedule only one patient per day, so you'll have a calm, comfortable experience with our full attention.
Step 3 — Feedback Session (60 minutes)
After testing, you'll meet with Dr. Lee to review your results in plain language. You'll have time to ask questions and understand what the findings mean for you and your life.
Step 4 — Written Report
Within approximately five weeks of your feedback session, you'll receive a thorough written report outlining your strengths, challenges, and personalized recommendations. With your permission, we can share this with teachers, physicians, or other providers.

We provide evaluations for patients across the lifespan — children ages 6 and up, adolescents, adults, and older adults. Early childhood evaluations are considered on a case-by-case basis.
-
In some cases, when concerns are very specific and a full evaluation isn’t necessary, Dr. Lee may offer a more targeted evaluation. Examples include ADHD-only assessments, dyslexia evaluations, and dementia or memory-specific evaluations for patients not covered under Medicare Part B.
These follow the same high standard of care as our comprehensive evaluations — the scope is simply more targeted to your specific concern.
Targeted evaluations aren’t the right fit for every situation, and Dr. Lee reviews each request individually to make sure you’re getting the level of assessment that will actually answer your questions. If you’re wondering whether a targeted evaluation might be appropriate for you or your child, just reach out — we’re happy to talk it through.
-
We accept cash, debit/credit cards, check, HSA, FSA, and HRA. We also offer CareCredit for flexible monthly payment options — a great choice if you’d prefer to spread the cost over time.
To reserve your appointment, one-third of the total cost is required at the time of booking, with the remaining balance due the day before your testing and feedback appointments.
-
A superbill is the document your insurance company needs to reimburse you for out-of-network services. It includes the procedure codes (CPT) and diagnosis codes (ICD-10) from your evaluation — essentially a detailed, insurance-ready receipt. We provide it automatically after your evaluation and will help you understand exactly how to submit it. You shouldn’t have to figure that out on your own.

Understanding the Cost of Your Evaluation
We believe knowing what to expect financially is part of feeling prepared, and we want that conversation to be straightforward, not stressful. Clary Clinic is a self-pay practice, which means we do not bill insurance directly. Payment is due at the time of service, and we accept HSA, FSA, and HRA funds in addition to standard payment methods.
We also know this is a significant investment, which is why we offer CareCredit, a healthcare credit card designed specifically for medical expenses. CareCredit allows you to pay for your evaluation over time through flexible financing plans, including options with little to no interest. Click the link below for additional information.
Why We Don't Accept Insurance
When a clinic bills insurance directly, insurance companies determine which tests can be given and how much time can be spent, often limiting the scope of the evaluation before it even begins. At Clary Clinic, we believe those decisions belong with you and your clinician, not your insurance provider.
Keeping our practice private pay means we can:
Take as much time as each person genuinely needs
Choose assessment tools based on clinical fit, not coverage rules
Use the most current and accurate methods available
Deliver a thorough evaluation that reflects your full picture, not an abbreviated one
Using Your Insurance Benefits
Not being billed directly by us doesn't necessarily mean insurance won't help. Many plans include out-of-network benefits for neuropsychological services, and after your evaluation we'll provide you with a superbill, a detailed receipt containing everything your insurance needs to process a reimbursement claim. Any reimbursement is paid directly to you.
Coverage varies widely by plan. Before your appointment, call the member services number on the back of your insurance card and ask:
Do I have out-of-network benefits for neuropsychological evaluations?
What is my out-of-network deductible, and how much has been met?
What percentage is reimbursed after the deductible is satisfied?
What do I need to submit a claim with a superbill?
You may also want to reference these billing codes when you call: 90791, 96132, 96133, 96138, 96139
